Primary Care Group of Physicians that promotes Health and Wellness not only to our own patients but also to seniors 55+ in the community. We provide In-Person Educational Programs and Classes, Nutritional Classes, Socializations, Fun and games (i.e. Bingo's Loteria' s, Arts and Crafts, Sewing Classes, Computer Classes, etc.). All is free to our seniors. Conviva / Humana provide everything that a senior needs to stay busy and safe from this ongoing Covid, we are still following Covid Protocols and Social Distancing. Conviva is also contracted to Silver Sneaker Exercise Classes/ Instructors through Humana. These classes are offered to our seniors so that they can benefit from Go365 with Humana and get credit for so many things that Conviva offers our seniors/patients.
